Getting Started
How to make an appointment?
We see self-referred patients, in addition to offering consultation and accepting referrals from other practitioners. Our practitioners are not primary-care providers.

New Patient
All new patients need to complete a New Patient Packet and may need to take other steps necessary to establish care. Please see "New Patient Checklist" for details.
Our seminars and talks are often "free" and also a great way to “get to know us” before scheduling a consultation. Check out the calendar of events for the dates/places of any talks or seminars. If none are presently listed, call our office to request notification of events. Your Initial Office Visit
Your initial visit will usually take between 1.5 to 2 hours and includes consultation with your provider and time for vital signs, testing, lab work as indicated, instruction and counseling. Hearing your concerns and discussing your priorities for your health is the primary focus. Knowing you is as important to us, as knowing the symptom or disease state you express.
In consideration of those with sensitivities, we would greatly appreciate you refraining from wearing perfumes and colongnes during your scheduled visit.
Nutritional Supplementation
Nutritional therapy is frequently an integral and valuable component of your treatment plan. We will help you select the correct nutrients, medications and bio-active substances for your condition. We recommend only high quality products. Screening and Laboratory Testing
After our office receives your past medical records and history form, we may recommend to you screening tests that would be helpful for your initial evaluation. Laboratory/diagnostic testing is a valuable component of your treatment plan. Test results are used to uncover the root causes of your medical condition and to design a personalized health and vitality program for you. We use a variety of laboratories including Oregon Medical Lab, Labcorp and some specialized reference laboratories.
Cost and Insurance Information The charge for an initial evaluation will vary depending upon complexity and time spent. The charge for a typical new patient visit is $340. Your provider may recommend additional diagnostic testing not included in the cost of the visit.
Established patient visits range between $117-$239. Generally, our rates fall at or below commercial
insurance allowed amounts and are very competitive for our area. Payment Options
Payment in full is expected on the day of service unless other arrangement have been made in advance with the business office. This requirement may be waived if you have insurance coverage and assign benefits to our clinic. You will be asked to pay for whatever portion of charges your insurance plan requires at the time of service. Please see our Financial Policy for details. We accept cash, checks, debit or credit cards (MasterCard, Visa, Discover, or American Express). We have available interest free credit payment plans for those who qualify. Insurance Billing If you have insurance and have signed an "Assignment of Benefits" statement to us, we will courtesy bill your primary insurance carrier for you. We are out of network providers for all commercial insurances. It is important to remember that your insurance is a contract between you and your insurer. Although we file claims as a courtesy to you, you are still responsible for payment of services regardless of the amount that your insurance pays. Medically necessary services are reimbursable according to individual insurance policies and exclusions.
Medicare/Medicare Advantage Plans
We do not participate with, or bill Medicare.
Telephone Follow-up Services If appropriate, telephone consultation is available for follow-up care. Telephone consultation is generally not covered by insurance, and payment in full via a credit card on file in our office is required unless other payment information and instructions are provided prior to your appointment.
Confirmation and Cancellation of Appointments
Due to the large investment in time that our office devotes to preparing and gathering data, reviewing your information and preparing for your visit, there is a 72 hour cancellation policy. A change in an appointment must be confirmed at least 72 hours prior to your scheduled consultation. Your credit card will be charged $100.00 for a missed appointment. You may change your appointment by calling our office. If calling after hours, please leave a message.
Phone Calls, Messages & Faxes Our office hours are Monday – Friday 9 am to 5 pm PST. We also close Noon to 1:00 pm Monday & Friday only. We are closed for all major holidays.
For your convenience, evening or early morning appointments are available for a small additional charge. Phone: 541-762-1155. Fax: 541-762-1154. If you call after hours, our office staff will return your call on the next business day. If you have a medical emergency, call 911 or go directly to the nearest emergency room. When leaving a message at our office, please be brief and include the following information: * Full name * Reason for call * Best time to be reached * Phone Number(s) * Email address, if desired If you prefer, or if email communication is more effective for you, our office has a “Consent to use E-mail communication” that you may e-mail, mail or fax to us. Prescription Refill Requests You can request prescription refills by calling your pharmacy. They will fax a request to us. Please plan ahead and allow 72 hours to process refills. If you medication request if for custom compounded medication please allow 10 days to avoid any interruptions in your medications. |
