New Patient Checklist

Welcome to Oregon Optimal Health. We feel privileged to help you with your health - it is an investment in your life and future.
 
 
Please use this checklist as a guide in preparation for your first appointment:
 
 
Complete the New Patient Packet
New Patient Packets can be obtained via e-mail or mail.
Allow ample time for completion - the packet contains an extensive questionnaire.
Return the completed packet to our office at least five days prior to your appointment.
Faxing is the preferred method of receipt.
Please provide a copy of your insurance card(s), front & back, if applicable.
  
 
Request records from your Primary Care Provider and/or other practitioner or specialist
We will need office visit, procedure or diagnostic records completed within the last two years.
You should request records directly from your providers - we cannot make the requests for you.
These records should be received into our office at least five days prior to your appointment.
Faxing is the preferred method of receipt
Be aware that it can take up to 10 days from request to receipt into our office of your records.
 
 
Day of Your Appointment
Plan on being at the office a minimum of 90 minutes.
If your appointment requires a physical exam you may bring a loose t-shirt and shorts that you
would feel comfortable having an exam in, rather than a patient gown/robe.
 
In consideration of those with sensitivities, we would greatly appreciate you refraining from wearing perfumes and colognes during your scheduled visit.
 
 
Prepare for Your Meeting With Our Practitioners:
The process of completing the New Patient Packet should help you focus and prioritize your list
of health concerns that you would like to have addressed.
 
 
Additional tips to help make the initial meeting productive and efficient:
 
Take notes as you work through the patient forms.
Bring the notes and/or a family member or trusted friend to help you.
Focus on your primary health concerns.
Prioritize specific health goals which you would like to obtain and be prepared to discuss
these with your practitioner.
 
 
Prepare yourself - the journey to improved health requires your active participation.
