Getting started
We see self-referred patients, in addition to offering consultation and accepting referrals from other practitioners.
It's important for your quality of care that we have as much medical information about you as is practical at your initial appointment.
To start, complete our secure Initial Contact Form. After that initial form, expect contact from our office, then additional forms including a Health Questionnaire, additional demographics and gathering of information from your other providers.
Insurance coverage
As a courtesy we bill most primary insurance plans for the covered services provided. Secondary billing may be possible, depending on the plan. Covered services generally include most office visits and many labs.
We can help you verify insurance benefits and eligibility in advance of your appointment. IIt's an option found in the Initial Contact Form). Doing this helps eliminate unexpected coverage issues. A benefits check is not a guarantee of coverage. Your insurance contract is an agreement between you and your insurer, and although we files claims as a courtesy for you, you are responsible for payment of services regardless of the amount that your insurance pays.
We ask you to pay your estimated portion of the visit at check-out, such as co-pay, coinsurance, deductible, or non-covered services.
Jeanette Welker, NP is in-network with most commercial plans including most Regence Blue Cross Blue Shield plans, the First Choice Health Network, the Pacific Source Network, the Providence Signature Network & Statewide Plans/Open Option, Providence Preferred Network, Meritain (via the Aetna POS II Network), MODA Connexus Network, Bridgespan, and most recently with the Aetna Choice POS II Network. Insurance networks can change frequently, and this list is not a guarantee of coverage. Each insurance company has some restrictive plans, so it is best to verify coverage individually.
Jeanette has opted out of Medicare. We offer a reduced "self pay" fee schedule for patients with Medicare or Medicare-type plans. Medicare cannot be billed with this self pay requirement, as mandated by Medicare. Medicare patients are also restricted from self-billing. Many Medicare plans do cover labs and prescriptions ordered by our provider.
We do not accept Oregon Health Plan.
Uninsured or have a high deductible plan? We offer substantial discounts for clients declining insurance billing, and have access to excellent low cost lab options..

Scheduling your appointment
We schedule appointments upon completion of all intake forms and once we've received any pertinent records from other providers.
If you have records from other providers, please use the release form above to request those records be sent to us. Recent labs - those completed within the last 6-12 months are the highest priority. Recent chart notes from other providers, and any special records or diagnostics are especially helpful. Functional Medicine labs of any age are appreciated.
Your initial office visit
Your initial new patient visit will usually last between 1 1/2 - 2 hours and includes consultation with your provider and time for vital signs, ordering possible lab work, customizing a treatment plan, instruction and counseling. Hearing your concerns and discussing your priorities for your health is the primary focus.
Its best to wear comfortable, loose clothing, and to be well hydrated for the biometric testing which is part of your initial examination.In consideration of those with sensitivities, we would greatly appreciate you refraining from wearing perfumes and colongnes during your scheduled visit.
Follow-up care
Your expectations, and our requirements for follow-up care are as varied as our clients.
Generally speaking, at minimum expect one follow-up visit after your initial evaluation to discuss lab results, effect of initial therapies suggested, and to formulate an individualized plan of care for your health concerns with your provider.
Many clients prefer or require multiple visits for multiple test results or questions. At the end of each visit we will provide written notes regarding your treatment plan, and can respond to minor follow-up questions and clarifications by phone or email. Complex questions or medical issues will require a return office visit. We can also provide some follow-up consultations by phone for a small fee, however, these telephone consults are generally not covered by insurance.
In short, at the minimum, annual visits and laboratory testing is necessary for continued care, but more frequent visits or testing is common.
